Unique Link For Each School
Each school has their own attestation form with a unique link. This will help with the sorting and managing of the data on a daily basis.
- Students who attend multiple sites (example: HHS and PSSC) will need to fill out a separate form for each site.
- Secondary students participating in athletics will need to fill out the athletics daily screening form.
Online Daily Attestation
Each school’s Daily Student Attestation Form asks the parent/guardian to complete the attestation at least 30 minutes prior to first bell time each morning. Please note: we do not have the ability to close the survey, and submissions can be made at any time.
We ask elementary students to name their teacher, but do not require secondary students to list each of their teachers.
There is no date section on the online form. The date is automatically recorded when the form is submitted. This requires parents/guardians to fill out the form the day the student is attending school.
- A parent/guardian or another caregiver (such as the childcare provider) should complete the form for younger students. Older students capable of answering the questions may fill it out for themselves. The first question asks for the email address of the person filling out the form that day.
- The attestation form is a google survey, but logging into a gmail account is NOT required. Anyone with the link can complete the form.
Paper Attestation Forms
Please encourage everyone to use the online form. We understand this may not an option for everyone, so please have paper forms available on site. The form, in English/Spanish, is linked above. There is a spot at the top for your schools’ paper form for a QR code to encourage everyone to fill out the form online.
Schools have two options for managing paper forms.
- If your staff has capacity, please input the data from paper forms into the google form. This will put all data in the spreadsheet staff must review each day.
- If staff do not enter the data manually, the forms are to be kept on file until we no longer need them under DAN# GS2020-009 Rev. 0. Please file the forms by date and keep them on file for 14 days; then shred them.
Daily Temperature Checks
Temperature checks should be done at home prior to attending school. Temperature checks will be done at school when:
- The student arrives at school without a completed attestation form.
- Students say are not feeling well when in line to enter school or they feel warm or feverish.
Non-touch digital forehead thermometers will be provided to each school for temperature checks.
Reviewing attestation Data
District staff ARE required to be logged into their district google account to view the google spreadsheet displaying attestation data.
A limited number of staff at each school have pre-loaded permission to view the spreadsheet:
- School administrators (principal)
- School office staff (office manager and office assistant)
- District health services staff (school nurse)
Staff should sort the data to review results and follow up with students who said "yes" to any of the questions. Here is a guide on how to filter data in a google spreadsheet.
