If you want to take home district-owned mobile technology this summer, please complete Form 8321F. Keep a copy for yourself, give a copy to your administrator and send a digital copy to the Help Desk.
Employees accept responsibility to have insurance coverage on these devices. Notify your homeowner's/renter's/automobile's insurance company to check your policy and make sure these items are covered if damaged or stolen from your home or car. This may require a rider on your insurance policy for coverage.
Per Board Policy 8321, the employee assumes safe transport of district-owned property and any theft or damage resulting from an accident or burglary is the employee's responsibility.
If the equipment has been lost or stolen, please follow these steps IMMEDIATELY:
- Contact the Security Department at 206-631-7600 (answered 24 hours). Provide as much of the following information as possible regarding the device: make, model, district barcode (if known), quantity and any other pertinent information. Make sure to follow up, if no one can assist when you call. TIP: Add the Security Department's phone number to your cell phone.
- Contact your manager or supervisor via email or phone about the loss.
- Notify the police in the city/county where the theft occurred. Note: The police report file number will be needed for the device to be tracked.
- MOST IMPORTANT: If student or staff personally identifiable data is on the lost device, the end user must contact Mark Finstrom, chief technology officer, at 206-631-3330.
